What is The Ageing Well Team?
The Ageing Well team gives older patients the support they need to look after their own health and wellbeing.
We provide emotional and practical support to patients aged 65+, enabling them to live independently in the community.
Our Co-ordinators work in a task-centred way, primarily with patients who may be at risk of hospitalisation or that require some additional support. With the support of the team, patients are able to regain confidence and have more control over their lives.
Our Ageing Well team is made up of Care Co-ordinators from various backgrounds and agencies meaning we have links to lots of different skill sets. Our Co-ordinators work collaboratively to utilise the expertise on offer which provides the patient with the most effective support when it is needed.
The team consists of Co-ordinators from:
- AgeUK Northamptonshire
- Northamptonshire Carers
- Northamptonshire Health Foundation Trust
- Adult Social Care Team with West Northamptonshire Council
- Social Prescribing Link Worker from the surgeries
Dr Chander Nayar based at Saxon Spires Practice is the Age Well/Frailty lead within Northamptonshire Rural PCN.
Chander joined the general practice training scheme at Northampton General and worked in a number of specialities at the hospital as well as another local practice. She did her part of the GP training at Saxon Spires. After completing training in 2010, she became a GP partner at Saxon Spires in 2011.
She did dedicated sessions looking after care home patients for well over 10 years, during this time she gained experience in looking after frail patients in the community. This led to developing a keen interest in managing frailty. This was consolidated further, through training she received at the University of Northampton, as part of the iCAN project.
She does this role one day a week and continues to be a GP at Saxon Spires. She has close working relationship with the various team members who are very valuable in helping our patients ‘Age Well’.
