What is Social Prescribing?

Social Prescribing is recognising that people’s health and wellbeing are determined mostly by a range of social, economic, and environmental factors. It seeks to address people’s needs in a holistic way. It also aims to support individuals to take greater control of their own health.

  • Support you to make positive changes in your life.
  • Help with housing, benefits and financial problems.
  • Refer/signpost you to community organisations such as support groups and creative activities.
  • Refer you to professional support.
  • Support you to communicate and/or identify barriers.
  • Listen and give you the time you need to talk.
  • Provide advice on jobs, training and education.
  • Research the relevant support available to you.
  • Advocate to help make sure your views are listened to and voice is heard.
